South Carolina Board of Occupational Therapy

Requirements for License Renewal/ Reinstatement


All occupational therapists and occupational therapy assistants must renew their licenses biennially on or before March 15 of every other year. Renewal application forms are mailed to all current licensees in January. The next renewal period will be March 15, 2011. At that time, licensees will be required to report 16 hours of Continuing Education, which will have been earned between March 16, 2009 and March 15, 2011. See Continuing Education.

If there is an address or name change at any time during the two-year period, notification is to be sent to the board office within 30 days of such change. A copy of the legal document effecting any name change is required as proof before the Board can change its records.


Application for License Renewal

Complete all questions on the application, sign and return with any accompanying explanations and fee. See Fee Schedule. Incomplete applications will be returned for corrections.

A person who fails to renew a license and practices as an occupational therapist or an occupational therapy assistant after March 15 is practicing without a license and is subject to penalties. If a licensee fails to renew by April 15, the license will automatically lapse.

A licensee who does not wish to renew an active license may select inactive status on the renewal form and return the form to the Board office no later than April 15. A licensee whose license is placed on inactive status may not practice occupational therapy while the license is inactive.

License Reinstatement Procedure

By law, no person shall practice as an occupational therapist or occupational therapy assistant without holding an active license in this state. Licenses not renewed or placed on inactive status as of April 15 following the renewal period, are considered lapsed. The reinstatement procedure is as follows:

An OT/OTA who is not currently licensed in another state and whose S.C. license has lapsed or been inactive for:

(1) Fewer than three years may reinstate the license by making application to the Board, paying the reinstatement fee, and submitting proof of 8 hours of continuing education for each full year that the license has been lapsed or inactive;

(2) At least three years, but less than five years, may reinstate the license by making application to the Board, paying the reinstatement fee, and satisfactorily completing 500 clinical practice hours under the on-site supervision of a S.C. licensed occupational therapist;

(3) At least five years, but less than ten years, may reinstate the license by making application to the Board, paying the reinstatement fee, satisfactorily completing 750 clinical practice hours under the on-site supervision of a S.C. licensed occupational therapist, and successfully completing a course(s) approved by the Board;

(4) At least ten years or more may reinstate the license by making application to the Board, paying the reinstatement fee, successfully passing an examination approved by the Board, and satisfactorily completing 1,000 clinical practice hours under the on-site supervision of a S.C. licensed occupational therapist.

An OT/OTA who is currently licensed in another state and whose S.C. license has lapsed or been inactive for:

(1) Fewer than three years may reinstate the license by making application to the Board, paying the reinstatement fee, submitting proof of completing 8 hours of continuing education for each full year that the license has been lapsed or inactive, and submitting official verification of all current licenses;

(2) Three or more years may reinstate the license by making application to the Board, paying the reinstatement fee, submitting evidence satisfactory to the Board of not less than 1,000 hours of out-of-state employment within the last year and official verification of all current licenses.
