How to Apply For a License

Application Forms

The computerized licensing examinations are administered by Prometric for nursing home administrator applicants and community residential care facility administrator applicants. Click Prometric to go to their website.

In accordance with Regulation 93-110 and 120:

The board administers examinations for both nursing home and community residential care administrators.

1. Nursing home administrators applicants sit for a two-part examination. The national portion is prepared by the National Association of Boards of Examiners for Long Term Care Administrators. The South Carolina portion is prepared by the South Carolina board and examines applicants on regulations approved by the Department of Health and Environmental Control as they relate to nursing homes.

2. Community residential care facility administrator applicants sit for a two-part examination. The national portion is prepared by the National Association of Boards of Examiners for Long Term Care Administrators. The South Carolina portion is prepared by the South Carolina Board relating to regulations promulgated by the Department of Health and Environmental Control as they relate to community residential care facilities.

Every applicant for licensure is required to pass the NAB examination at a scale score of 113, and pass the state examination at a raw score of 75 percent.

The Board provides a report of their scores to applicants who complete an examination. Applicants are notified of the results of each examination by mail only.

An applicant who is taking the national and South Carolina portions of the examination, and who receives a passing score in either portion will be entitled to receive credit for the portion passed and may be re-examined on the portion failed.

An applicant who has failed the examination twice must petition the board to pursue licensure. An applicant who has successfully complied with the requirements of the licensing law and the standards, passed the examination, and paid the fees for the initial licensure period will be issued a license as a nursing home administrator or as a community residential care administrator.

The certificate of licensure is issued by the Board, certifying that the applicant has met the requirements of the law and regulations entitling the applicant to serve, act and practice as a duly licensed nursing home administrator or community residential care administrator. A license cannot be transferred to another individual.
